If I am interested in weight loss, what does that look like?

All weight loss clients must book an initial consultation ($75) with us to discuss your needs and goals. Once your consultation is complete we will create a care plan for you to meet your needs + goals. Anyone interested in prescription assisted weight-loss must meet the BMI requirements.

How are appointments completed?

With the exception of IV therapy, all appointments are telephone calls initiated by your provider at Revive (we will call you). While we strive to adhere to scheduled appointment times, please allow a 15 minute window for your call to be initiated.

Do you accept insurance?

We are a cash pay practice. We do not accept or file with insurance.

How does your IV service work?

We are a mobile IV service covering several areas of Texas. If you are interested in our IV therapy services, text or call and we will check provider availability in your area. Once confirmed, we will collect your $100 deposit (that will be applied to your service total) and schedule your appointment.

What do i need to do before an appointment?

Once you have reached out to schedule an appointment with us, you will need to complete the patient intake and consent forms that will be emailed to you. If you are scheduling a weight loss consultation appointment, you will need to review/become familiar with the education document that will be in that email as well.

What if we need to reschedule?

We require at least 24 hours notice to cancel your appointment. We understand emergencies happen, but late cancels will result in a $50 charge to your card. We are willing to reschedule without the fee on a case by case basis.
